Annual Information Form (AIF)
Every season FVSRA must have a current Annual Information Form on file for each participant. This form is required to be in the possession of FVSRA before a participant can attend programs. These forms are updated every Summer and are good until the following Summer season.
Please make sure that you inform FVSRA immediately of any medication or health changes. In order for FVSRA to provide the best possible service, it is imperative that your AIF reflects any adjustments concerning your health.
