Special Olympics Med Application
Special Olympics Illinois requires that each participant must have a current Medical Application in order to participate in Special Olympics sports. This form requires an original signature by a Physician. This form is good for two years from the date of the Physician’s signature. If you are unsure when you need to renew a medical application please check with Mike Younie, Athletics Coordinator (630) 907-1114.
